Uncertain significance for Tuberous sclerosis 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000548.5(TSC2):c.4562C>G (p.Pro1521Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TSC2 gene (transcript NM_000548.5) at coding-DNA position 4562, where C is replaced by G; at the protein level this means replaces proline at residue 1521 with arginine — a missense variant. Submitter rationale: This sequence change replaces proline with arginine at codon 1521 of the TSC2 protein (p.Pro1521Arg). The proline residue is highly conserved and there is a moderate physicochemical difference between proline and arginine. This variant is not present in population databases (ExAC no frequency). This variant has been reported in an individual in the Leiden Open-source Variation Database (PMID: 21520333). However, in that individual a pathogenic allele was also identified in TSC2, which suggests that this c.4562C>G variant was not the primary cause of disease. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0").
