NM_000051.4(ATM):c.3856del (p.Cys1286fs) was classified as Pathogenic for ATM-related cancer predisposition by ClinGen Hereditary Breast, Ovarian and Pancreatic Cancer Variant Curation Expert Panel, ClinGen, citing ClinGen HBOP ACMG Specifications ATM V1.3.0. This variant lies in the ATM gene (transcript NM_000051.4) at coding-DNA position 3856, deleting one base; at the protein level this means shifts the reading frame starting at cysteine residue 1286, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The c.3856del p.Cys1286Alafs*7 variant in ATM is a frameshift variant in a biologically-relevant-exon predicted to cause a premature stop codon leading to nonsense mediated decay in a gene in which loss-of-function is an established disease mechanism. This alteration results in a termination codon upstream of the most C-terminus pathogenic alteration (ATM p.Arg3047*), as classified by the HBOP VCEP, and is expected to be more deleterious. This variant has been reported in at least one individual with Ataxia-Telangiectasia (PMID: 26896183). This variant is absent from gnomAD v4.1.0. In summary, this variant meets criteria to be classified as pathogenic for autosomal dominant ATM-related cancer predisposition and autosomal recessive Ataxia-Telangiectasia based on the ACMG/AMP criteria applied as specified by the HBOP Variant Curation Expert Panel.
