Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000249.4(MLH1):c.2265del (p.Arg755fs), citing Ambry Variant Classification Scheme 2023: The c.2265delG pathogenic mutation, located in coding exon 19 of the MLH1 gene, results from a deletion of one nucleotide at nucleotide position 2265, causing a translational frameshift with a predicted alternate stop codon (p.R755Sfs*28). This alteration occurs at the 3' terminus of theMLH1 gene, is not expected to trigger nonsense-mediated mRNAdecay and results in the elongation of the protein by 25 amino acids. This frameshift impacts the last two amino acids of the native protein. However, premature stop codons are typically deleterious in nature and the impacted region is critical for protein function (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this variant is interpreted as a disease-causing mutation.
