Pathogenic — the classification assigned by GeneDx to NM_005188.4(CBL):c.1199T>G (p.Met400Arg), citing GeneDx Variant Classification (06012015). This variant lies in the CBL gene (transcript NM_005188.4) at coding-DNA position 1199, where T is replaced by G; at the protein level this means replaces methionine at residue 400 with arginine — a missense variant. Submitter rationale: This variant is denoted as p.Met400Arg (p.M400R) at the protein level, c.1199T>G at the cDNA level, and results in the change of a Methionine for an Arginine in exon 8 of the CBL gene (NM_005188.2). According to the Catalogue of Somatic Mutations in Cancer (COSMIC) database, the M400R missense mutation in the CBL gene has been reported previously as a somatic mutation in haematopoietic and lymphoid tissue. The M400R amino acid substitution is non-conservative with a neutral and non-polar residue (Met) being replaced by a positively charged and polar residue (Arg) at a position of the protein that is highly conserved. Missense mutations in nearby codons (D390Y and R420Q) have been reported in association with Noonan-like syndrome (Martinelli et al., 2010). Also, missense mutations in nearby codons (C396R, H398R, C404R, W408R) have been been reported in association with juvenile myelomonocytic leukemia (JMML) (Niemeyer et al., 2010). The variant is found in NOONAN panel(s).
