Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_024675.4(PALB2):c.2748+2T>C, citing Ambry Variant Classification Scheme 2023. This variant lies in the PALB2 gene (transcript NM_024675.4) at the canonical splice donor site of the intron immediately after coding-DNA position 2748, where T is replaced by C; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.2748+2T>C intronic variant results from a T to C substitution two nucleotides after coding exon 7 in the PALB2 gene. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6497 samples (12994 alleles) with coverage at this position. This nucleotide position is highly conserved in available vertebrate species. Using two different splice site prediction tools, this alteration is predicted by BDGP to abolish the native donor splice site, but is predicted to weaken (but not abolish) the efficacy of the native donor splice site by ESEfinder; however, direct evidence is unavailable. Alterations that disrupt the canonical splice donor site are typically deleterious in nature (ACMG Recommendations for Standards for Interpretation and Reporting of Sequence Variations. Revision 2007. Genet Med. 2008;10:294). As such, the c.2748+2T>C variant is classified as likely pathogenic.
