Uncertain significance — the classification assigned by GeneDx to NM_001378454.1(ALMS1):c.8533A>G (p.Ile2845Val), citing GeneDx Variant Classification (06012015): A variant of uncertain significance has been identified in the ALMS1 gene. The I2846V variant has not been published as pathogenic or been reported as benign to our knowledge. This variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server; Exome Aggregation Consortium). Nevertheless, the I2846V variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. Additionally, this substitution occurs at a position where amino acids with similar properties to isoleucine (I) are tolerated across species, and valine (V) is tolerated in several species. Furthermore, in silico analysis predicts this variant likely does not alter the protein structure/function.
