Uncertain significance — the classification assigned by GeneDx to NM_001018116.2(CAVIN4):c.161T>G (p.Ile54Arg), citing GeneDx Variant Classification (06012015): A variant of uncertain significance has been identified in the MURC gene. The I54R variant has not been published as pathogenic or been reported as benign to our knowledge. It is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The I54R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. Although this substitution occurs at a position where amino acids with similar properties to isoleucine (I) are tolerated across species, in silico analysis predicts this variant is probably damaging to the protein structure/function. Nonetheless, this variant lacks observation in a significant number of affected individuals, segregation data, and functional evidence, all of which would further clarify pathogenicity.
