NM_201596.3(CACNB2):c.385C>T (p.His129Tyr) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the CACNB2 gene (transcript NM_201596.3) at coding-DNA position 385, where C is replaced by T; at the protein level this means replaces histidine at residue 129 with tyrosine — a missense variant. Submitter rationale: The H75Y variant of uncertain significance in the CACNB2 gene has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. H75Y was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, nor was it observed in the Exome Aggregation Consortium, indicating it is not a common benign variant in these populations. The H75Y variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. However, this substitution occurs at a position that is not conserved. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function.
