Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_002439.5(MSH3):c.1654-2_1654delinsTAAG, citing Ambry Variant Classification Scheme 2023: The c.1654-2_1654delAGAinsTAAG variant, located in intron 11 and coding exon 12 of the MSH3 gene, results from an in-frame deletion of three nucleotides and insertion of 4 different nucleotides at nucleotide positions c.1654-2 to c.1654. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). These nucleotide positions are highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data). Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as likely pathogenic.
