NM_000263.4(NAGLU):c.1021+1G>A was classified as Likely pathogenic for Mucopolysaccharidosis, MPS-III-B by Women's Health and Genetics/Laboratory Corporation of America, LabCorp, citing LabCorp Variant Classification Summary - May 2015: Variant summary: NAGLU c.1021+1G>A is located in a canonical splice-site in the last intron and is predicted to affect mRNA splicing resulting in a significantly altered protein due to either exon skipping, shortening, or inclusion of intronic material. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing and loss of NAGLU function. Several computational tools predict a significant impact on normal splicing: Four predict the variant abolishes a 5' splicing donor site. However, these predictions have yet to be confirmed by functional studies. The variant was absent in 249766 control chromosomes. To our knowledge, no occurrence of c.1021+1G>A in individuals affected with Mucopolysaccharidosis Type IIIB (Sanfilippo Syndrome B) and no experimental evidence demonstrating its impact on protein function have been reported. At least one variant downstream of this position in the last exon (c.1241A>G, p.His414Arg) has been classified as Pathogenic by our lab, suggesting a critical role of last exon inclusion in NAGLU protein function. No submitters have cited clinical-significance assessments for this variant to ClinVar. Based on the evidence outlined above, the variant was classified as likely pathogenic.
