Likely pathogenic for Glutaric aciduria, type 1 — the classification assigned by Women's Health and Genetics/Laboratory Corporation of America, LabCorp to NM_000159.4(GCDH):c.263G>C (p.Arg88Pro), citing LabCorp Variant Classification Summary - May 2015: Variant summary: GCDH c.263G>C (p.Arg88Pro) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 251424 control chromosomes (gnomAD). c.263G>C has been observed in at least one individual affected with Glutaric Acidemia Type 1 (Schuurmans_2023). Two different variants affecting the same codon have been classified as likely pathogenic/pathogenic by our lab (c.262C>T, p.Arg88Cys; c.262C>A, p.Arg88Ser), supporting the critical relevance of codon 88 to GCDH protein function. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 37020324, 38137040). No submitters have cited clinical-significance assessments for this variant to ClinVar. Based on the evidence outlined above, the variant was classified as likely pathogenic.
