Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Color Diagnostics, LLC DBA Color Health to NM_000179.3(MSH6):c.3951_3953delinsAAAGCAAGTTATTC (p.His1317fs), citing ACMG Guidelines, 2015. This variant lies in the MSH6 gene (transcript NM_000179.3) at coding-DNA position 3951 through coding-DNA position 3953, replacing the reference sequence with AAAGCAAGTTATTC; at the protein level this means shifts the reading frame starting at histidine residue 1317, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This variant deletes 3 nucleotides and inserts 14 new nucleotides in exon 9 of the MSH6 gene, creating a frameshift and premature translation stop signal. This variant is predicted to escape nonsense-mediated decay and be expressed as a truncated protein. Although functional studies have not been reported, this variant is expected to disrupt the ATPase domain and the MSH2 binding domain (PMID: 9564049, 9774676, 10636912, 22232658). To our knowledge, this variant has not been reported in individuals affected with MSH6-related disorders in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of MSH6 function is a known mechanism of disease (clinicalgenome.org). Based on the available evidence, this variant is classified as Pathogenic.
