NM_003413.4(ZIC3):c.973G>A (p.Glu325Lys) was classified as Likely Pathogenic for Heterotaxy, visceral, 1, X-linked; VACTERL association, X-linked, with or without hydrocephalus by Illumina Laboratory Services, Illumina, citing ICSLVariantClassificationCriteria RUGD 01 April 2020. This variant lies in the ZIC3 gene (transcript NM_003413.4) at coding-DNA position 973, where G is replaced by A; at the protein level this means replaces glutamic acid at residue 325 with lysine — a missense variant. Submitter rationale: The ZIC3 c.973G>A p.(Glu325Lys) missense variant has not, to our knowledge, been reported in the peer-reviewed literature. This variant is not observed in version 2.1.1 or version 3.1.2 of the Genome Aggregation Database. The p.(Glu325Lys) variant is located in the nuclear export signal domain, which is important for appropriate cellular localization and trafficking of ZIC3 (PMID: 17185387). Multiple lines of computational evidence suggest the variant may impact the gene or gene product. This variant has been shown to segregate with disease in this proband's family in individuals with phenotypes consistent with ZIC3-related disorders. Based on the available evidence, the c.973G>A p.(Glu325Lys) variant is classified as likely pathogenic for ZIC3-related disorders.
