This variant lies in the LZTR1 gene (transcript NM_006767.4) at the canonical splice donor site of the intron immediately after coding-DNA position 400, where T is replaced by C; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.400+2T>C intronic variant results from a T to C substitution two nucleotides after coding exon 4 in the LZTR1 gene. This variant was reported in individual(s) with features consistent with LZTR1-related schwannomatosis/Noonan syndrome (Ambry internal data). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this variant is likely pathogenic for an increased risk of LZTR1-related schwannomatosis (SWN) and would be expected to cause autosomal recessive Noonan syndrome when present along with a second pathogenic or likely pathogenic variant on the other allele; however, the association of this alteration with autosomal dominant Noonan syndrome is unlikely.
