This variant lies in the ANTXR2 gene (transcript NM_058172.6) at coding-DNA position 134, where T is replaced by G; at the protein level this means replaces leucine at residue 45 with arginine — a missense variant. Submitter rationale: The L45R variant in the ANTXR2 gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The L45R variant was not observed in approximately 6,200 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The L45R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. A missense variant in the same residue (L45P) has been reported in the Human Gene Mutation Database in association with hyaline fibromatosis syndrome (Stenson et al., 2014), supporting the functional importance of this region of the protein. The L45R variant is a strong candidate for a pathogenic variant, however the possibility it may be a rare benign variant cannot be excluded.
