Uncertain significance — the classification assigned by GeneDx to NM_001148.6(ANK2):c.11380C>T (p.Pro3794Ser), citing GeneDx Variant Classification (06012015). This variant lies in the ANK2 gene (transcript NM_001148.6) at coding-DNA position 11380, where C is replaced by T; at the protein level this means replaces proline at residue 3794 with serine — a missense variant. Submitter rationale: A novel variant of uncertain significance has been identified in the ANK2 gene. The P3794S variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. It was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Furthermore, the P3794S variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. Nevertheless, this substitution occurs at a position that is not conserved across species, and no missense variants in nearby residues have been reported in the Human Gene Mutation Database (Stenson et al., 2014), indicating that this region of the gene is not known to harbor disease-causing variants. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function.Therefore, based on the currently available information, it is unclear whether this variant is pathogenic or rare benign.
