NM_000054.7(AVPR2):c.445C>T (p.Arg149Cys) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the AVPR2 gene (transcript NM_000054.7) at coding-DNA position 445, where C is replaced by T; at the protein level this means replaces arginine at residue 149 with cysteine — a missense variant. Submitter rationale: The R149C variant in the AVPR2 gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The R149C variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The R149C variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position where amino acids with similar properties to Arginine are tolerated across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. We interpret R149C as a variant of uncertain significance.
