NM_020975.6(RET):c.2307delinsTCGAGACCTGCTGTCAGAGTTCAACGTGAGCTT (p.Leu779fs) was classified as Pathogenic for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.2333_2334insGAGCTTCGAGACCTGCTGTCAGAGTTCAACGT (p.L779Sfs*12) alteration, located in exon 13 (coding exon 13) of the RET gene, consists of an insertion of GAGCTTCGAGACCTGCTGTCAGAGTTCAACGT at position 2333, causing a translational frameshift with a predicted alternate stop codon after 12 amino acids. This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. Loss-of-function variants in RET are known to cause Hirschsprung disease; however, such associations with Multiple Endocrine Neoplasia Type-2 (MEN2) have not been observed (Amiel, 2001; Wagner, 2012). Based on the supporting evidence, this alteration is pathogenic for Hirschsprung disease; however, the association of this alteration with MEN2 is unlikely. This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). Based on the available evidence, this alteration is classified as pathogenic.
