Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_006231.4(POLE):c.5878_5879dup (p.Asp1960fs), citing Ambry Variant Classification Scheme 2023: The c.5878_5879dupGA variant, located in coding exon 43 of the POLE gene, results from a duplication of GA at nucleotide position 5878, causing a translational frameshift with a predicted alternate stop codon (p.D1960Efs*40). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. Loss-of-function variants subject to nonsense mediated decay (NMD) in POLE are known to cause POLE deficiency; however, such associations with POLE-related polymerase proofreading-associated polyposis (PPAP) have not been reported. Based on the supporting evidence, this alteration is pathogenic for POLE deficiency; however, the association of this alteration with POLE-related PPAP is unknown.
