NM_006231.4(POLE):c.656_657del (p.Val219fs) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.656_657delTG variant, located in coding exon 7 of the POLE gene, results from a deletion of two nucleotides at nucleotide positions 656 to 657, causing a translational frameshift with a predicted alternate stop codon (p.V219Gfs*16). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. Loss-of-function variants subject to nonsense mediated decay (NMD) in POLE are known to cause POLE-deficiency; however, such associations with POLE-related polymerase proofreading-associated polyposis (PPAP) have not been reported. Based on the supporting evidence, this alteration is pathogenic for POLE-deficiency; however, the association of this alteration with POLE-related polymerase proofreading-associated polyposis (PPAP) is unknown.
