Uncertain Significance for Leukoencephalopathy with brain stem and spinal cord involvement-high lactate syndrome — the classification assigned by Broad Center for Mendelian Genomics, Broad Institute of MIT and Harvard to NM_018122.5(DARS2):c.748C>G (p.Leu250Val), citing ACMG Guidelines, 2015: The p.Leu250Val variant in DARS2 has been reported in 1 individual with leukoencephalopathy with brain stem and spinal cord involvement-high lactate syndrome (PMID: 33977142), and has been identified in 0.0008% (9/1179914) of European (non-Finnish) chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP rs1024219171). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. In vitro functional studies provide some evidence that the p.Leu250Val variant may slightly impact protein function (PMID: 33977142). However, these types of assays may not accurately represent biological function. Computational prediction tools and conservation analyses suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In summary, while there is some suspicion for a pathogenic role, the clinical significance of this variant is uncertain.
