This variant, to our knowledge, has not been reported in the medical literature in relation to PIK3CA-Related overgrowth disorders and is absent from the general population (gnomAD v.4.1.0), indicating it is not a common variant. Other variants in the same codon have been reported in a somatic state in numerous types of cancer in the cancer database COSMIC (Genomic Mutation IDs: COSV55875400, COSV55942600). Multiple variants in the same codon, p.Gln546Arg, p.Gln546Leu, p.Gln546Lys, p.Gln546His, and p.Gln546Glu have been reported in affected individuals and are considered pathogenic (ClinVar Variation IDs: 45466, 375899, 13657, 376491, 13654). The PIK3CA gene is defined by the ClinGen Brain Malformations expert panel as a gene that has a low rate of benign missense variation and where pathogenic missense variants are a common mechanism of disease (Lai et al., PMID: 35997716). Based on available information and an internally developed protocol informed by the ACMG/AMP guidelines for variant interpretation and gene-specific practices from the ClinGen Criteria Specification Registry (Leon-Quintero FZ et al., PMID: 39434542), the PIK3CA c.1637_1638inv (p.Gln546Pro) variant is classified as likely pathogenic.
