Uncertain significance for Ectodermal dysplasia-syndactyly syndrome 1 — the classification assigned by Kasturba Medical College, Manipal, Kasturba Medical College, Manipal, Manipal Academy of Higher Education, Manipal, India to NM_030916.3(NECTIN4):c.374A>C (p.Tyr125Ser), citing ACMG Guidelines, 2015. This variant lies in the NECTIN4 gene (transcript NM_030916.3) at coding-DNA position 374, where A is replaced by C; at the protein level this means replaces tyrosine at residue 125 with serine — a missense variant. Submitter rationale: A novel missense, c.374A>C in exon 2 of NECTIN4 was observed in homozygous state in proband. On segregation analysis, the variant was found to be in heterozygous state in his parents. This variant is not present in heterozygous and homozygous state in population database gnomAD and our in-house database of 2819 exomes. In silico prediction tools (CADD_Phred, REVEL, MutationTaster, ClinPred) are consistent in predicting the variant to be damaging to the protein function. The clinical features observed in him are in concordance with ectodermal dysplasia-syndactyly syndrome 1. Thus, the above mentioned variant in homozygous state is interpreted to be the probable cause for the condition observed in him.
