A presumed nomenclature of c.(?_-487)_(45+1_46-1)del has been designated for the purposes of this classification. The exact breakpoint at the 5' end of this variant is unknown, therefore this deletion may extend upstream of the annotated region of this gene. Although the exact breakpoints of this deletion are not known, it is predicted to remove the initiation codon and result in an absence of protein or a truncation of the encoded protein due to translation initiation at a downstream site. The variant allele was found at a frequency of 8.3e-06 in 120776 control chromosomes in the gnomAD database (Structural Variants v4.1 dataset). Deletion of the first exon in the GCK gene has been reported in the literature in multiple individuals affected with Monogenic Diabetes (e.g. Osbak_2009, Gao_2024). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
