This variant lies in the NOTCH3 gene (transcript NM_000435.3) at coding-DNA position 125, duplicating one base; at the protein level this means shifts the reading frame starting at cysteine residue 43, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The NOTCH3 c.125dup; p.Cys43LeufsTer32 variant (rs749829137), to our knowledge, is not reported in the medical literature or gene specific databases. This variant is also absent from the Genome Aggregation Database (v2.1.1), indicating it is not a common polymorphism. This variant causes a frameshift by inserting a single nucleotide, so it is predicted to result in a truncated protein or mRNA subject to nonsense-mediated decay. Most pathogenic NOTCH3 variants occur in exons 2-24 and either create or destroy a cysteine residue within an EGF-like domain, truncating variants are not associated with CADASIL (Rutten 2014).
