Pathogenic for Developmental and epileptic encephalopathy, 1; Autosomal dominant nonsyndromic hearing loss 65 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001199107.2(TBC1D24):c.1384del (p.Glu462fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TBC1D24 gene (transcript NM_001199107.2) at coding-DNA position 1384, deleting one base; at the protein level this means shifts the reading frame starting at glutamic acid residue 462, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Glu462Serfs*61) in the TBC1D24 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 98 amino acid(s) of the TBC1D24 protein. This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with autosomal recessive TBC1D24-related conditions (PMID: 27281533). This variant disrupts a region of the TBC1D24 protein in which other variant(s) (p.Ala515Val) have been determined to be pathogenic (PMID: 20727515, 26668325, 27281533; internal data). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
