Likely pathogenic for Distal myopathy with posterior leg and anterior hand involvement; Myofibrillar myopathy 5; Hypertrophic cardiomyopathy 26 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001458.5(FLNC):c.7718T>G (p.Ile2573Ser), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the FLNC gene (transcript NM_001458.5) at coding-DNA position 7718, where T is replaced by G; at the protein level this means replaces isoleucine at residue 2573 with serine — a missense variant. Submitter rationale: This sequence change replaces isoleucine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 2573 of the FLNC protein (p.Ile2573Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of FLNC-related conditions (internal data). In at least one individual the variant was observed to be de novo. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on FLNC protein function. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
