Pathogenic for Dilated cardiomyopathy 1HH; Myofibrillar myopathy 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004281.4(BAG3):c.4dup (p.Ser2fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the BAG3 gene (transcript NM_004281.4) at coding-DNA position 4, duplicating one base; at the protein level this means shifts the reading frame starting at serine residue 2, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Ser2Lysfs*17) in the BAG3 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in BAG3 are known to be pathogenic (PMID: 21353195, 25008357). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with BAG3-related conditions. For these reasons, this variant has been classified as Pathogenic.
