Uncertain significance for Neuropathy, hereditary sensory and autonomic, type 2A; Pseudohypoaldosteronism type 2C — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_018979.4(WNK1):c.1900G>A (p.Ala634Thr), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces alanine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 634 of the WNK1 protein (p.Ala634Thr). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with WNK1-related conditions (PMID: 32790646). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt WNK1 protein function with a negative predictive value of 95%. Experimental studies have shown that this missense change does not substantially affect WNK1 function (PMID: 32790646). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
