Uncertain significance for Pulmonary fibrosis and/or bone marrow failure, Telomere-related, 4; Dyskeratosis congenita, autosomal recessive 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002582.4(PARN):c.872T>G (p.Leu291Trp), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces leucine, which is neutral and non-polar, with tryptophan, which is neutral and slightly polar, at codon 291 of the PARN protein (p.Leu291Trp). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with PARN-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PARN protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
