NM_001458.5(FLNC):c.3556G>A (p.Ala1186Thr) was classified as Uncertain significance for Distal myopathy with posterior leg and anterior hand involvement; Myofibrillar myopathy 5; Hypertrophic cardiomyopathy 26 by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces alanine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 1186 of the FLNC protein (p.Ala1186Thr). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with FLNC-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on FLNC protein function. This variant disrupts the p.Ala1186 amino acid residue in FLNC. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 21520333, 26436962; Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
