This variant lies in the AP4E1 gene (transcript NM_007347.5) at 3 bases upstream of the translation start (5' untranslated region), where G is replaced by T. Submitter rationale: A variant of uncertain significance has been identified in the AP4E1 gene. The c.-3 G>T sequence change in the 5' untranslated region of the AP4E1 gene has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The c.-3 G>T variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). This variant alters a conserved position in the Kozak sequence, which plays a role in the initiation of protein translation, but is not expected to alter the ATG initiation codon. Several in silico models predict c.-3 G>T does not impact splicing. Other regulatory mutations have not been reported to our knowledge. However, in the absence of RNA/functional studies, the actual effect of this sequence change in this individual is unknown. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic mutation or a rare benign variant.
