NM_024596.5(MCPH1):c.145C>G (p.His49Asp) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015): A variant of uncertain significance has been identified in the MCPH1 gene. The H49D variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The H49D variant was not observed with any significant frequency in approximately 5,900 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project. The H49D variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species and in silico analysis predicts this variant is probably damaging to the protein structure/function. However, missense variants in nearby residues have not been reported in the Human Gene Mutation Database in association with MCPH1-related disorders (Stenson et al., 2014). Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic variant or a rare benign variant.
