Pathogenic — the classification assigned by GeneDx to NM_000088.4(COL1A1):c.3208G>A (p.Gly1070Ser), citing GeneDx Variant Classification (06012015). This variant lies in the COL1A1 gene (transcript NM_000088.4) at coding-DNA position 3208, where G is replaced by A; at the protein level this means replaces glycine at residue 1070 with serine — a missense variant. Submitter rationale: The G1070S pathogenic variant in the COL1A1 gene has been reported previously in association with osteogenesis imperfecta; however, data was unavailable for review (Zhao D, et al., 2014; Song Y et al., 2014). G1070S occurs in the triple helical domain and replaces the Glycine in the canonical Gly-X-Y repeat. Variants in these Glycines result in poor winding of the collagen triple helix and a less functional protein.The G1070S variant was not observed in approximately 64000 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The G1070S variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. Other nearby missense variants affecting Glycine residues (G1061D, G1064A, G1076C/S, G1079S) have been reported in the Human Gene Mutation Database in association with osteogenesis imperfecta (Stenson et al., 2014), supporting the functional importance of this region of the protein. Therefore, the presence of this pathogenic variant is consistent with the diagnosis of a COL1A1-related skeletal dysplasia.
