This variant lies in the PDHA1 gene (transcript NM_000284.4) at coding-DNA position 858 through coding-DNA position 861, duplicating 4 bases; at the protein level this means shifts the reading frame starting at arginine residue 288, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The NM_000284.3:c.858_861dup (p.Arg288LeufsTer10) change is a frameshift variant in PDHA1 gene. This variant is predicted to result in nonsense-mediated decay (NMD). In total, 6 individuals are diagnosed with PDHA1-related Pyruvate dehydrogenase complex (PDHc) deficiency (MIM #312170). These include 6 females. Among them, 4 cases have confirmed de novo occurrence. The variant has been reported in 3 published cases (PMIDs: 15384102, 24718837, 27290639). Additional 3 unpublished cases from internal data are included. Last literature search: July 12, 2024. This variant is absent or extremely rare in population-based cohorts in the Genome Aggregation Database (gnomAD). Individuals harboring this variant present with clinical features compatible with PDHA1-related PDHc deficiency. In summary, this variant meets criteria to be classified as pathogenic (P) for PDHA1-related PDHc deficiency based on the ACMG/AMP criteria applied: PVS1, PS2, PM2, PM7 (last assessment October 15, 2024).
