This variant lies in the HUWE1 gene (transcript NM_031407.7) at coding-DNA position 12680, where T is replaced by C; at the protein level this means replaces leucine at residue 4227 with serine — a missense variant. Submitter rationale: The L4227S variant in the HUWE1 gene has not been published as a pathogenic variant, nor has it been reported as a benign polymorphism to our knowledge. The L4227S variant was not observed in approximately 6500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The L4227S variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position within the HECT domain that is conserved across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. Based on review of recent literature and internal GeneDx data in the context of the 2015 ACMG standards and guidelines for the interpretation of sequence variants (Richards et al., 2015), we now interpret L4227S as a pathogenic variant,
