Uncertain significance for X-linked Alport syndrome — the classification assigned by 3billion to NM_033380.3(COL4A5):c.4481C>T (p.Ser1494Phe), citing ACMG Guidelines, 2015. This variant lies in the COL4A5 gene (transcript NM_033380.3) at coding-DNA position 4481, where C is replaced by T; at the protein level this means replaces serine at residue 1494 with phenylalanine — a missense variant. Submitter rationale: The variant is not observed in the gnomAD v4.1.0 dataset. Predicted Consequence/Location: Missense variant In silico tool predictions suggest damaging effect of the variant on gene or gene product [REVEL: 0.93 (>=0.6, sensitivity 0.68 and specificity 0.92); 3Cnet: 0.97 (>=0.6, sensitivity 0.72 and precision 0.9)]. The same nucleotide change resulting in the same amino acid change has been previously reported to be associated with COL4A5-related disorder (PMID: 8940267). However, the evidence of pathogenicity is insufficient at this time. Therefore, this variant is classified as VUS according to the recommendation of ACMG/AMP guideline.
