NM_005993.5(TBCD):c.1757C>T (p.Ala586Val) was classified as Pathogenic by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TBCD gene (transcript NM_005993.5) at coding-DNA position 1757, where C is replaced by T; at the protein level this means replaces alanine at residue 586 with valine — a missense variant. Submitter rationale: This sequence change replaces alanine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 586 of the TBCD protein (p.Ala586Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with TBCD-related conditions (PMID: 27807845, 28158450). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on TBCD protein function. For these reasons, this variant has been classified as Pathogenic.
