Likely pathogenic for Combined immunodeficiency due to ZAP70 deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001079.4(ZAP70):c.1690T>C (p.Cys564Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 564 of the ZAP70 protein (p.Cys564Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with severe combined immunodeficiency (PMID: 18509675). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects ZAP70 function (PMID: 33482200, 36078131). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
