Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002936.6(RNASEH1):c.442T>C (p.Cys148Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 148 of the RNASEH1 protein (p.Cys148Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with progressive external ophthalmoplegia (PMID: 23596069, 28508084, 30340744). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt RNASEH1 protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic.
