Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000249.4(MLH1):c.1409+1_1409+6del, citing Ambry Variant Classification Scheme 2023: The c.1409+1_1409+6delGTATGG intronic variant, located in intron 12 of the MLH1 gene, results from a deletion of 6 nucleotides within intron 12 of the MLH1 gene. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). These nucleotide positions are well conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site; however, direct evidence is insufficient at this time (Ambry internal data). Variants that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this variant is classified as likely pathogenic.
