Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_024675.4(PALB2):c.1838del (p.Gln613fs), citing Ambry Variant Classification Scheme 2023: The c.1838delA pathogenic mutation, located in coding exon 5 of the PALB2 gene, results from a deletion of one nucleotide at nucleotide position 1838, causing a translational frameshift with a predicted alternate stop codon (p.Q613Rfs*15). This alteration was reported in a cohort of 152 unselected Czech patients with pancreatic ductal adenocarcinoma and not in 1226 healthy controls (Borecka M et al. Cancer Genet, 2016 May;209:199-204). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
