Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000186.4(CFH):c.220C>T (p.Leu74Phe), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces leucine, which is neutral and non-polar, with phenylalanine, which is neutral and non-polar, at codon 74 of the CFH protein (p.Leu74Phe). This variant is present in population databases (rs368835171, gnomAD 0.007%). This missense change has been observed in individual(s) with clinical features of hemolytic uremic syndrome and/or C3 glomerulopathy (PMID: 29888403). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change does not substantially affect CFH function (PMID: 34189567). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
