This variant lies in the LDLR gene (transcript NM_000527.5) at the canonical splice donor site of the intron immediately after coding-DNA position 1060, where T is replaced by G; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.1060+2T>G variant in LDLR has been reported in 1 individual with coronary artery disease (Khera 2016) and 3 individuals with familial hypercholesterolemia (FH, ClinVar submission accession: SCV000484781.1). It has also been identified in 3/30612 of South Asian chromosomes by gnomAD (http://gnomad.broadinstitute.org). This frequency is low enough to be consistent with the frequency of FH in the general population. This variant occurs within the canonical splice site (+/- 1,2) and is predicted to cause altered splicing leading to an abnormal or absent protein. Loss of function of the LDLR gene is an established disease mechanism in autosomal dominant FH. In summary, although additional studies are required to fully establish its clinical significance, this variant meets criteria to be classified as likely pathogenic for autosomal dominant FH.
