Pathogenic for Early-infantile DEE — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_172107.4(KCNQ2):c.471G>A (p.Trp157Ter), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the KCNQ2 gene (transcript NM_172107.4) at coding-DNA position 471, where G is replaced by A; at the protein level this means converts the codon for tryptophan at residue 157 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: This sequence change creates a premature translational stop signal (p.Trp157*) in the KCNQ2 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in KCNQ2 are known to be pathogenic (PMID: 14534157, 23692823, 27779742). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with KCNQ2-related conditions (PMID: 23692823, 31418850). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 369744). For these reasons, this variant has been classified as Pathogenic.
