Uncertain significance for Charcot-Marie-Tooth disease axonal type 2C — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021625.5(TRPV4):c.1858G>T (p.Val620Phe), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine, which is neutral and non-polar, with phenylalanine, which is neutral and non-polar, at codon 620 of the TRPV4 protein (p.Val620Phe). This variant is present in population databases (no rsID available, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with TRPV4-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on TRPV4 protein function. This variant disrupts the p.Val620 amino acid residue in TRPV4. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 2046044, 18587396, 20037588, 21573172). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing.
