Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_000719.7(CACNA1C):c.2695G>A (p.Asp899Asn), citing Ambry Variant Classification Scheme 2023: The p.D899N variant (also known as c.2695G>A), located in coding exon 20 of the CACNA1C gene, results from a G to A substitution at nucleotide position 2695. The aspartic acid at codon 899 is replaced by asparagine, an amino acid with highly similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. According to data from gnomAD, the frequency for this variant is above the maximum credible frequency for a cardiac disease-causing variant in this gene based on internally established thresholds (Karczewski et al. Nature. 2020 May;581(7809):434-443; Whiffin et al. Genet Med. 2017 10;19:1151-1158). Based on the available evidence, the clinical significance of this variant remains unclear for CACNA1C-related neurodevelopmental disorder; however, it is unlikely to be causative of CACNA1C-related long QT syndrome / Timothy syndrome.
