Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_003998.4(NFKB1):c.1049A>G (p.Tyr350Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 350 of the NFKB1 protein (p.Tyr350Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with common variable immunodeficiency (PMID: 33995346). It has also been observed to segregate with disease in related individuals. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt NFKB1 protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change affects NFKB1 function (PMID: 33995346). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
