Likely pathogenic for Familial X-linked hypophosphatemic vitamin D refractory rickets — the classification assigned by Women's Health and Genetics/Laboratory Corporation of America, LabCorp to NM_000444.6(PHEX):c.1949T>C (p.Leu650Pro), citing LabCorp Variant Classification Summary - May 2015: Variant summary: PHEX c.1949T>C (p.Leu650Pro) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 183434 control chromosomes (gnomAD). c.1949T>C has been observed in individuals affected with X-Linked Hypophosphatemic Rickets (Jo_2020, Constantacos_2021). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Internally validated machine learning-based Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt protein function with a positive predictive value of at least 95%.
