Uncertain significance for Birt-Hogg-Dube syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_144997.7(FLCN):c.1394T>G (p.Phe465Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces phenylalanine, which is neutral and non-polar, with cysteine, which is neutral and slightly polar, at codon 465 of the FLCN protein (p.Phe465Cys). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with FLCN-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt FLCN protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
